Mobile QR Code Opt-In Proof & Verification
Official hosted proof document fulfilling Twilio requirements for the "Mobile QR code" consent method.
1. Image of Marketing Material Carrying the QR Code
In beauty and barber shops, ChairCheck provides an in-shop counter sign / table tent display carrying the QR code. Clients scan this QR code with their mobile phone camera to check into the live walk-in queue or book future appointments.

2. Destination URL That the QR Code Leads To
When scanned, the QR code leads directly to the public ChairCheck Client Portal and Opt-In web page:

On this destination page, the client can view open appointment slots, view the live walk-in waitlist, and optionally request a single-use verification code (OTP) and transaction reminders.

• The square HTML checkbox is optional and unchecked by default.
• An explicit "Skip SMS & Browse as Guest" button allows consumers to decline messaging and still use business services (preventing forced consent).
